Linking subsequent workitems in a doc is not done by the toolbar's indent button, but by the tab key.

AFAIK headings do not need/cannot be indented, as the first child workitem in a doc is automatically linked to it.

I did not try it myself, but the necessary field would be "updatedBy".
But this may be different for workitems, wiki, pages, documents
